Currently accepting appointment requests. Elizabeth is licensed in Ohio, Pennsylvania, Maryland and Virginia. That means she is able to do telehealth visits with anyone but you must be physically present anywhere in one of those states during the visit, or come to our office. Please include contact information and type of insurance you are using or indicate that you are private pay if applicable. DO NOT include insurance numbers at this time, just the name please. What happens after you join our waitlist?

-Your information is added to our waitlist, and we create your chart in our electronic medical record (Elation).

-You will receive an email with the intake forms from Elation.

-Once your intake forms have been completed and reviewed, you will receive an email with a link to schedule your appointment.

-After scheduling your appointment, you will receive an invitation to sign up for Elation’s Patient Portal, which will allow you to view your records, see your appointments and contact us with any questions.
