Currently accepting appointment requests. Elizabeth is licensed in Ohio, Pennsylvania, Maryland and Virginia. That means she is able to do telehealth visits with anyone but you must be physically present anywhere in one of those states during the visit, or come to our office. Please include contact information and type of insurance you are using or indicate that you are private pay if applicable. DO NOT include insurance numbers at this time, just the name please. You will be notified when scheduling becomes available. Please add potsmedicalcenter as an accepted address to your email service. You should receive a confirmation email upon submission. Please check your spam if it does not appear. If you agree to a cash visit or we are credentialed with your insurance, you should receive a an email with a link to schedule an appointment within a week.
