Cash-Pay Information
While we continue the insurance credentialing process, appointments are currently available on a cash-pay basis.
Current Self-Pay Fees:
- Comprehensive Dysautonomia Evaluation (up to 2 hours): $400
- Transfer Evaluation (up to 90 minutes): $300
- Johns Hopkins Transfer Visit (up to 60 minutes): $200
- Established Follow-up (up to 30 minutes): $150
- Extended Follow-up (up to 60 minutes): $250
We are actively pursuing contracts with multiple insurance carriers. Because credentialing timelines vary and are outside our control, we cannot guarantee future participation with any specific insurance plan or guarantee reimbursement for out-of-network services.
Patients choosing self-pay will receive a superbill upon request that may be submitted to their insurance company for possible out-of-network reimbursement.

Understanding Your Superbill & Insurance Reimbursement
At Dysautonomia Management Center, we are currently considered an out-of-network provider
for all insurance plans. Payment is due at the time of your visit. Depending on your insurance
benefits, you may be eligible to receive reimbursement directly from your insurance company.
After each visit, we will provide you with a superbill, which is an itemized receipt containing the
medical information your insurance company may require to process an out-of-network claim.
Looking for an easier way to submit your claim?
Many of our patients choose to use Reimbursify®, a third-party service that helps simplify the
claim submission process. Instead of completing insurance forms yourself, you can upload your
superbill through the Reimbursify app, and they will submit the claim to your insurance company
on your behalf.

Please note:
Reimbursement is not guaranteed and depends entirely on your individual insurance plan and
out-of-network benefits.
Any fees charged by Reimbursify are set by Reimbursify and are separate from Dysautonomia
Management Center.
We are happy to provide the documentation needed for your claim but cannot determine your
insurance coverage or reimbursement amount.
Before Your First Visit:
We recommend contacting your insurance company and asking:
Do I have out-of-network benefits? What is my out-of-network deductible?
What percentage of covered charges will be reimbursed after my deductible is met?
Is prior authorization required for outpatient office visits?
Our team is happy to answer questions about your superbill or provide additional documentation
if requested by your insurance company.
